The below video is a vibration analysis case study where there was a coupling bolt failure on a 4000HP ID Fan motor on a fluid drive. This was originally presented at the 2016 Reliability, Process, & Maintenance Symposium (RPM) in Kalamazoo, Michigan on September 13-14, 2016.

Tom Spettel is a Senior Reliability Engineer at HECO. He is an ANSI/ISO Certified, Category IV Vibration Analyst. Tom’s background includes serving as an US Army Medical Specialist. He then worked in a chemical plant/refinery supervising inspections on large steam turbines, compressors, and pumps. Later, he became a vibration analyst, where he saw the similarities to the medical field.
